HBPC Registered Nurse (RN) is responsible for providing in home patient centered car and case management to assigned patients. RN functions as team leader/charge nurse providing leadership that facilitates delivery of high-quality nursing care through the use of the nursing process. Works effectively with all levels of staff to include collaborative efforts with other health professionals. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
