Phoenix Children’s Hospital is seeking an Advanced Practice Practitioner (APP) to join our growing heart transplant team. This position will be one of four APPs providing both inpatient and outpatient coverage to the pediatric heart transplant program. In this position, you will be working alongside a close group of two heart transplant cardiologists, APPs, one nurse coordinator, and two medical assistants. You will also collaborate closely with a dedicated nutritionist, pharmacist, social worker as well as others on the healthcare team in the care of these patients. You will also work closely alongside and in direct communication with patients and families daily. This busy program follows approximately 150 heart failure/transplant patients and performs between 12-16 transplants per year. The pediatric heart transplant program is a part of the Phoenix Children's Hospital Transplant Center which also includes programs in bone marrow, kidney and liver transplantation.
Pediatric Nurse Practitioner (Primary Care, Acute Care or Dual Certification) or Physician Assistant licensure
Certification in area of educational training
Previous inpatient clinical experience is preferred but not required, specifically in an environment providing care to transplant patients or congenital heart disease
General responsibilities include:
The team of four APPs provide inpatient and outpatient coverage to heart transplant patients Monday through Friday. This position will work rotating 8-hour days in conjunction with another part-time APP as a job share.
Includes H & Ps, consults, and rounding on patients (primarily in the Cardiovascular Intensive Care Unit “CVICU”) for pre transplant, transplant and post-transplant patients
Discharge education and coordination of care is extensive in this role
Requires management of medically complex pediatric patients with advanced heart failure, mechanical circulatory support, as well as post-transplant care including immunosuppressant management
You will be working alongside attending physicians as well as many other disciplines within transplant so good communication and interpersonal skills are required
This position partners with three other (APPs) to provide 24/7/365 call coverage to the heart transplant program for organ offers and transplant
Clinic is located in the Heart Center at Phoenix Children's Hospital on the campus at 1919 E. Thomas
Must be able to work autonomously and in partnership with multiple disciplines constantly
Management of the transplant population including evaluations and consenting for transplant, as well as follow up and coordination of ongoing care
Outpatient management of advanced heart failure and patients with Mechanical Circulatory Support
Patient and family education is extensive in this position
Supervising and working directly with dedicated transplant Medical Assistants and Registered Nurses
Active coordination and participation in committee meetings related to transplant patient management
