The Licensed Practical Nurse/Licensed Vocational Nurse (LPN/LVN) assigned to the VA Portland Health Care System (VAPORHCS) Specialty Clinics, under the Division of Hospital and Specialty Medicine (DHSM), provides outpatient clinical services for the subspecialties such as Medicine, Surgery, and Neurology. The duties of this position include but are not limited to the following: Functions as a team member under the guidance of a Registered Nurse. Adaptable and functions with minimal direction with the ability to prioritize and adjust care in response to clinic flow needs. Performs patient centered care to Veterans throughout the lifespan. Completes clinical reminders and medication reconciliation as clinically indicated. Provides medical assistive care and administration support when needed. Demonstrate the ability to observe, identify, and respond to the patient's need for nursing care recognizing deviations from baseline. Medication administration. Wound care treatments. Home meter downloading. Urological procedures. Suture and staple removal. Capillary blood glucose test. Equipment assisted care. Electrocardiograms. Cast removal. Dermatological procedures. Use of safe patient handling equipment. Responsible for patient education and documentation in nursing clinics and medication administration. Deploy proactive customer service and active listening. Engage in rapport building, limit setting, and de-escalation as applicable. Demonstrate flexibility and will be expected to work in multiple clinics and areas within duty station and with multiple services as needed to meet patient volume and acuity demands. Use of both hands and fingers required. Ability to distinguish basic colors, near vision correctable at 13" to 16" and far vision correctable in one eye to 20/20 and 20/40 in the other.
