This Renal Physician Assistant is located in the Medical Service at the Jamaica Plain campus location. This is a full-time 40 hour per week position. The Physician Assistant (PA) is accountable for and provides comprehensive wellness care, as well as acute, episodic, and chronic care, of adult clients. He/she holds a scope of practice and practices in collaboration with a physician. The PA provides and oversees the care of renal patients under the supervision of the Medical faculty. The PA will be well organized and able to manage a busy panel of his or her own patients along with providing oversight and coordination of care of patients in the additional MD run clinics and hemodialysis unit. Duties included but not limited to: The PA uses the nursing process and evidence-based practice to work collaboratively with the Renal Medical staff and expanded team (family/caregiver, internal and community-based services and disciplines involved in providing care to the patient) in developing the patient-driven holistic care plan. The PA provides diagnosis and treatment and assistance with care, diagnosis driven teaching and preparation for the next level of care as needed. In addition, the PA will participate in various quality assurance activities as needed. Applies the nursing process (assessment, diagnosis, outcome identification, planning, implementation, and evaluation) to systems or processes at the practice level to provide patient-driven holistic care. Provide nursing and medical services to Veterans and their families/caregivers in both individual and group settings. Emphasizes health promotion and disease prevention; diagnose and manage acute, chronic and episodic illnesses. Conducts independent PA out-patient clinics delivering general nephrology care to Veterans and reviews patient management with the collaborating physician as appropriate. Provides continuity of care for the MD run clinics including patient communication, medication management, ordering and interpreting tests and imaging as appropriate and assuring the patient has proper follow up for abnormal findings. Manages the patients in the onsite out-patient hemodialysis unit. Acts as VA liaison for veterans who dialyze in the community. Participates in the evaluation and coordination of care of patients for vascular access placement pre-hemodialysis. Coordinates the workup of renal transplant candidates and monitors compliance with the transplant waiting list requirements. Responsible for ordering infusions as needed for all renal patients including iron, blood transfusions, monoclonal antibodies and other treatments based on patient needs. Teaches and counsels Veterans, families/caregivers and groups. This includes patient education regarding options related to dialysis, including kidney transplant, and reviewing factors that are involved in the progression of renal disease. Demonstrates leadership by involving the Team and expanded Team in providing customized, evidence-based patient-driven care. Preferred Experience: Experience as an PA in the management of patients with renal disease in the out-patient and/or in-patient settings, including experience with dialysis and transplantation. Relevant recent experience as an PA in the management of patients with diseases commonly encountered in our renal patient population such as diabetes, hypertension, and coronary artery disease. Experience in the care of geriatric patients. Ability to evaluate, investigate and treat patients independently. Experience in collaborating and communicating effectively with other health care providers. Familiarity with the use of electronic medical records. Experience in the use of telemedicine and conducting virtual clinics. Work Schedule: This is full-time 40 hours per week. Compressed/Flexible: May be authorized. Telework: Not Available.
