The Transplant Coordinator coordinates solid o rgan (heart, lung, heart-lung, liver, kidney, pancreas, and multi-organ) transplantation of allpediatric and adult organ recipients. The Transpl antCoordinatorprovides follow-up and care across the continuum from pre- transplant,during transplant and post transplant. S/he collaborates wit hmembersof the health care team to enhance patient care by developing,su pporting and educating nursing staff. S/he is a clinician, educator,inve stigator, consultant and leader. S/he makes clinical judgments basedupon a synthesis of current research, concepts, principles,theories,and outco mes to solve complex problems. The Transplant Coordinator is anintegralm ember of a larger interdisciplinary team contributing toexcellence inpat ient care, research, teaching, and in providingleadership to the organiz ation.
Clinical Practice1. Provides a dvanced nursing care for patients before and aftersolid-organ transplant ation including heart, kidney, kidney-pancreas,liver and lung along the continuum of outpatient to inpatient 24 hours aday, 7 days a week.2.Coor dinates evaluation process for pre-transplant (referral)patients toensur e effective use of hospital and clinical resources.3. Assesses post-tra nsplant organ function and general health statusand collaboratively dete rmines need for additional assessment orintervention.4. Facilitates the scheduling of studiesor proceduresfordiagnostic purposes utilizing fiscal responsibility for patient andhospital costs.5. Participatesin the development and implementation of protocoland standards of carefor the t ransplant patient in collaboration withthe multidisciplinary team.6. As sists nurses and other staff to assess, plan, implement andevaluate the care of the transplant patient.7. Monitors the process and outcomes of routine care to determinequality improvement need.8. Provideson-call co verage, including coordination of activitiesprior to transplant, and as medical triage for out patients incollaboration with the physician on-ca ll.9. Coordinates transplantation process when a donor is available (24 hours/7 days a week) including communication between physicians, localor gan procurement organization, patient/families and the medicalteaminvolv ed including ED, OR, ICU (SICU, ACU), blood bankandhistocompatilbility l abs.10. Completes necessary documentation according to theUnited Networ kof Organ Sharing (UNOS) guidelines including listing, status changes an dpost-transplant follow-up information.11. Communicates with multidisci plinary team members and patients andfamilies in a timely manner in rega rd to patient care/issues.12. Collaborates with PRM and other team memb ers in coordinatingdischarge plans for patients pre-transplant or post-t ransplant includingreferral to home health / IV therapy / Hospice / Reha b / SNF.13. Communicates with insurer case manager on an as needed basi s.14. Refers patients or families for assistance utilizinghealthsystem and community resources as appropriate.15. Maintains patient transplant record with current data tofacilitate communication.16.Plans nursing ca re and medical procedures with fiscalresponsibility by workingcollaborat ivelywith transplantadministrators, financial coordinators, DUH utilizat ion review andthird-part case managers.Teacher/Educator1. Educates pati ents along the continuum to assist understanding ofprocesses and proced ures surrounding pre- and post-transplant issues.2. Develops patient ed ucation materials or identifies externalresources thatare appropriate fo r patient education in all phases oftransplantation care.3. Educates nu rses and other multidisciplinary team members aboutpatient care issues t o promote positive outcomes for transplantpatients.4.Provides education and consultation about transplantation, andorgan donations for the Duke Health System, public organizations orreferral centers/practices.5. Ser ves as a resource person/preceptor in organ transplant forgraduate and u ndergraduate nursing programs.6. Participates or leads team conferences , rounds, or meetings inrelation to care of the transplant patient or pr ogram issues.Research Utilization and Scholarly Inquiry1. Interprets re search and published data to nursing staff andcolleagues concerning curr ent issues in transplantation including thoseof a clinical, legal, ethic al and economic nature.2. Incorporates research relating to the care of transplant patientsinto evidence-based practice while collaboratively d eterminingappropriateness with the medical team.3.Identifies areas for s tudy and participates in and/or designs andconducts research.4. Dissemi nates findings or information through professionalpublications and/or pr esentations.Consultant1. Serves as a consultantto nurses and other heal th careprofessionals regarding the care of the organ transplant patient. 2. Advises referring physicians/nurses in the care of patients incollab oration with the surgical and medical teams.3.Serves asa community cons ultation resource through media,telephone andtransplant internet links f or information abouttransplantation and organdonation.4. Consults with unit leadership to resolve issues relating totransplant patients.Profess ional Leadership and Development1. Knowledge, Skills and Abilities
Attention to detail and e ffective communication and documentationwithnumerous physicians, organ p rocurement agencies, Operating Room (O.R.),Intensive Care Unit (ICU) and Emergency Department (ED) staff, thepatient and the patient's family ar e required.
BSN required, MSN preferred.
5 years nursing experience with at least two years in transplant, critical care, disease specific OR minimum of 3 years clinical nursing experience in transplant, critical care, disease specific
Degrees, Licensures, Certifications
Current or compact RN licensure in the state of North Carolina required. BLS required.
